We got the application yesterday and got all registered for the training. When looking through the application, I started to think there was no possible way that we could get it all done in two weeks or less. I mean, this thing usually takes people more time than that! So, during naptime yesterday and then again last night, I set out to do as much as I could. I must say, I am quite impressed by how much I got done. I think deadlines are good for me, especially when I want something so very badly.
Here's my list of things to get done, and the bolded are what we have finished in the last 24 hours or so.
1. Adoption Study Application Form
2. Individual Fact Sheet (2)
3. Adoption Study Questionnaire (this part is SUPER lengthy) (2)
4. Fee Disclosure Statement and Acknowledgement Form
5. Adoption Services Contract
6. Birth Parent Services Fee Agreement
7. Authorization for Release of Information (must be notarized)
8. Background Check Packet (still being sent to us)
9. Medical Form (2) - not due yet
10. Client Confidentiality Statement
11. Adoption Program Service Plan
12. Adoptive Family Data Sheet
13. Ben's birth certificate copy - called and is being sent out
14. Kari's birth certificate copy
15. Copy of marriage certificate
16. Verification of Health Insurance - called, and is being sent out
17. Reference letters (not due yet)
18. Copy of most recent income tax return
Oof. Oh, and we also need to get going on some online training that needs to be completed before we attend the training in the cities. Lots to do!
Yes LOTS to do - but you're doing super great in getting things done early and getting going! I'm so excited for you and Ben and your new family through this amazing blessing of adoption!
